Farmington Youth Assistance (FYA) is seeking nominations for its 36th Annual Youth Recognition Awards. We are looking for YOUTH or YOUTH GROUPS who deserve recognition for their extensive service, but who may not normally receive it from the community at large. One of the goals of Farmington Youth Assistance is to encourage positive examples of youth volunteerism. In order to honor these deserving youth we must locate them, and for that, we rely on you.
Nominations can be submitted for an INDIVIDUAL or a GROUP.
To be eligible for nomination, youth or group must…
• Reside in Farmington or Farmington Hills, attend Farmington Public Schools, or perform community service in these communities.
• Attend grades 6 through 12.
• Exhibit exemplary volunteer service through their community, faith-based organization or school totaling a minimum of 100 hours. Paid services performed and community service for high school credit such as National Honor Society or the courts are NOT eligible. Length and quality of commitment and individual initiative will be considered.
• The judgment of the Selection Committee is final.
Nominators should complete the Nomination Form for each youth or group nominated. The more detailed information given about the youth or the group, the more likely the youth or the group will be selected for this award.

The awards will be presented remotely this year, so we will be in touch with recipients and nominators about the presentations.
Nomination form must be submitted by Sunday, April 4th at 11:59pm to be eligible for this year’s awards.
